How Miriam Haxton, MD (Miriam Haxton) handles the mobile number and consent you provide for text messages. Effective August 16, 2026. Your health information is additionally protected under HIPAA and our Notice of Privacy Practices.
We keep clinical content in text messages to a minimum. Detailed health information is delivered through the secure patient portal.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors in support services, such as customer service, is permitted. All other use-case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
To deliver messages we use Hero EMR (Soaper LLC), our HIPAA-compliant practice platform, which acts as our service provider under a business associate agreement, and its telecommunications carriers. These providers may process your number only to deliver messages on our behalf.
You can opt out at any time by replying STOP, changing your setting in the patient portal (portal.miriamhaxtonmd.com), or contacting us. Reply HELP or contact us at (773) 688-8536 / miriam.haxton@miriamhaxtonmd.com for assistance.
Consent records and message logs are retained as part of your medical record for as long as required by law and our record-retention policy, and are protected with administrative, technical, and physical safeguards.
