Text message (SMS) consent

Miriam Haxton, MD sends text messages only to patients — or their parents or guardians — who have opted in. This page shows how patients opt in and the exact disclosure they agree to.

How patients opt in

Intake and communication-preference forms. New and existing patients complete Miriam Haxton, MD's intake paperwork on paper in the office or electronically through the patient portal. The form includes the optional text-message consent checkbox shown below. Leaving it unchecked has no effect on care.
Patient portal settings. Signed-in patients can turn text messages on or off at any time under Notifications → Text messages at portal.miriamhaxtonmd.com. The same disclosure is shown next to the toggle.
In person or by phone. A patient may ask our staff to enroll their mobile number. Staff read the disclosure below, and the patient's consent and the date are recorded in the patient's communication preferences in our electronic medical record.

Consent is never pre-checked, is not required to book an appointment or receive care, and can be withdrawn at any time by replying STOP, changing the portal setting, or telling our staff.

Message frequency varies and is typically 1 to 10 messages per month, mostly around scheduled visits. We do not send marketing or promotional text messages. Message and data rates may apply.

Opting out and getting help

Reply STOP to any message to opt out; you will receive one confirmation text and no further messages. Reply HELP for help, or contact us at (773) 688-8536 or miriam.haxton@miriamhaxtonmd.com. You can also turn texts off in the patient portal or ask our staff.
